A flat, well-lit shot of the printed problem with the whole question in frame. Glare, heavy shadow, extreme angles, and cropped-off text are what cause misreads. If a photo comes out wrong, retake it or type the problem instead.

Does it read diagrams and tables in the picture, or only text?

It reads the figure along with the text, which is the point for physics and chemistry problems where the given values live in the diagram. Very small labels can still be missed, so check the restated problem before trusting the answer.

Yes. Batch mode on paid plans takes a set of images and returns a worked page for each one, which is how most people clear a full worksheet instead of feeding it in a question at a time.
